Anyone interested in learning more about collaborative, data-driven solutions for addressing the long-term health effects caused by various social determinants of health.


Social determinants of health have been defined as conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. Social determinants include things like food security, housing, transportation, education, crime, pollution, and discrimination.

Early Bird Summit Registration ($249/person) Ends: Monday, April 30, 2018 (or when event capacity is reached)


Regular Summit Registration ($299/person): Ends: September 9, 2018 (or when event capacity is reached)


Late Registration Availability ($349/person): Beginning September 10, 2018 or sooner if event capacity has been reached, we will open a standby list for those who have not yet purchased a ticket. We will offer late-registration tickets if/when space becomes available.


 

Registration fees include all conference materials, access to conference sessions and breakout discussions, attendance at our October 8th networking reception, and continental breakfast and lunch on October 8th and 9th.

 


Should you need to cancel your registration for any reason - the deadline to do so is 6:00 p.m. Eastern Time on September 21, 2018. If cancellations are not received by this time, you will be held financially responsible for any and all existing conference fees. Written notifications are accepted via email and fax. Cancellation requests via voicemail are not accepted.

Cancellations made by the deadline of 6:00 p.m. Eastern Time on September 21, 2018: If payment has been submitted, you will receive a refund for the amount paid; however, please note that a $50.00 processing fee will be deducted.

Cancellations made after the deadline will not be issued a refund.

  • Email subject line/fax reference line: 2018 National Summit Registration Cancellation Request
  • Full name and contact information of the registrant (phone number and email address)
  • Reason for cancellation (Please indicate if you are a presenter or representative of a Coalition Partner)


The Root Cause Coalition welcomes all participants. Please contact our Manager of Education & Programming, Monique Brigham, at contact@rootcausecoalition.org or 202-266-2615 to discuss specific disability accommodations needed to participate fully in this event.


The main program will begin around 8:00am on Monday, October 8, 2018. However, we have added two sessions on Sunday, October 7, 2018 from 2:30pm- 5:30pm. We encourage all attendees to join us for our Sunday Institutes (included in registration): 

  • The Food is Medicine Institute for Providers, Payors and Policy Leaders
  • Place-based Investing for Health Institute for Providers, Payors and Policy Leaders


The summit will conclude around 5:00pm on Tuesday, October 9, 2018. Information regarding agenda specifics will be posted as the Summit nears.
