Person-Centered Care and Care Planning in Adult Day Health Care

Who Should Attend
This half-day, person-centered care planning seminar is designed for ADHC program directors, nursing home administrators, nursing staff, and social workers who contribute to, or responsible for, care planning in adult day health care.
All ADHC providers must implement person-centered care planning to comply with the federal HCBS Settings Rule and to overturn the presumption that ADHC is an institutional setting. This seminar will focus on how to operationalize the federal requirements, theory and concepts of person-centered care planning in adult day health care. Through case studies and small group work, attendees will create pertinent care plans with individualized interventions. A discussion with troubleshooting and critique will follow.
Continuing Education Credit
Licensed Nursing Home Administrators: This program is sponsored by the Foundation for Long Term Care (FLTC). The FLTC is a certified sponsor of professional continuing education with the National Association of Boards of Examiners for Long Term Care Administrators (NAB). This program has been submitted for 4.0 hours of continuing education credit for nursing home administrators under its sponsor agreement with NAB/NCERS. State licensure boards, however, have final authority on the acceptance of individual courses. Credit is awarded upon request via credit form and verification of the hours attended. Attendance is reported directly to NAB within 30 days of the program. Certificates will be available via your NAB CE Registry.
Licensed Social Workers: LeadingAge New York, Inc., SW CPE is recognized by the New York State Education Department’s (NYSED) State Board for Social Work as an approved provider of continuing education for licensed social workers. This program offers up to 4.0 hours of continuing education for Licensed Master Social Workers (LMSW) and Licensed Clinical Social Workers (LCSW). Credit is awarded upon request via credit form and verification of the hours attended. Credit reports will be available on our website approximately 30 days following the program.

Check payments can be accommodated when necessary, however; registrations will not be complete until payment is received. Full payment must be received by the date of the program to access to the materials and ensure participation.
Cancellations received by September 4 are refundable, less a 25 percent administrative fee. Those who are registered and cancel after that date or do not attend the event will not receive refunds and are still responsible for payment in full if not already paid.
